Spring Immunization Clinic
Platte County Health DepartmentDear Parent/Guardian,

Keeping your student immunized is important to your child’s health and each year the Platte County Health Department provides hundreds of immunizations to students from all over Platte County. The Platte County Health Department is partnering with Park Hill South High School to provide all required and recommended immunizations for students entering 12th grade on April 16, 2020.

The vaccines being offered are:
 Meningococcal/Menactra (Meningitis) [Required]
 Tdap/Boostrix (Tetanus, Diphtheria, and Pertussis) [Required]
 Human Papillomavirus (HPV) [Recommended]
 Hepatitis A [Recommended]

If you would like your student to receive any of these vaccines, please complete the, BTS Clinic Registration Consent and Screening Form 20-21.pdf and attach a copy of your insurance card. Once they are completed, please return them with your student to give to the school nurse. You must complete and return the Consent and Registration Form and Student Screening Form at least two weeks before your scheduled vaccination clinic. Students who qualify for the clinic are those who are uninsured, underinsured, have Medicaid, are Native American/Native Alaskan, or who have private insurance which covers vaccines. We currently accept Cigna, United Healthcare, and Blue Cross and Blue Shield. Please check with your insurance provider to see if your plan covers these vaccines. If you have any further questions, feel free to contact the health department at 816-587-5998. Please encourage your student to have breakfast the day of the clinic.
