2017-18 Kidguard Insurance Enrollment Form

Park Hill School District annually offers parents/guardians the opportunity to enroll their child/children in a low cost insurance program. As per Board of Education policy JHA, “The Park Hill Board of Education recommends that all students have accident insurance for both their own and their parents’/guardians’ protection. Although arranging for such insurance is the responsibility of the student and parents/guardians, the Board will name an insurance carrier each year to offer group rates. Participation in the group plan is optional. Parents/guardians and students will deal directly with the insurance carrier.” The District is offering coverage through Kidguard Insurance. Coverage becomes effective at 11:59 PM on the postmark date of the enrollment envelope or the date payment is received in Kidguard Insurance’s office, whichever is the earlier date.
