Medication


The Medication Guideline Authorization Form must be completed and signed by the parent/guardian in order for medications to be given at school.

All medications, prescription or over-the-counter, must be presented in the original container along with the completed medication guideline form before the medication can be dispensed in the health clinic.

Any change in the time or dosage of medication must be accompanied by a written request from the physician. No expired medication will be dispensed.

It is the student’s responsibility to come to the health clinic for assistance in taking medication. High school students may carry one (1) dose of prescription or over-the-counter medication as long as it is in the original labeled container. The student should have their parent/guardian permission and be responsible for taking the needed dose. Parents are encouraged to notify health personnel when their student is carrying medication. Medication kept in the health office will be under lock and key.

If your student will need special assistance or additional monitoring to take their medication at school, please contact the health services staff to discuss this.

PARK HILL SOUTH HIGH SCHOOL WILL NOT PROVIDE ANY MEDICATION FOR STUDENTS. THIS INCLUDES ASPIRIN, TYLENOL, COUGH DROPS, DECONGESTANTS OR ANTACIDS.
